Vendor Application For:
AMS PTSO Holiday Vendor Fair ~ Saturday, December 6, 2014 ~ 9 to 3
Entry fee for 10 ft by 10 ft space inside Ansonia Middle School Gymnasium is $35. We do not guarantee electricity for your
space, but will try to accommodate if requested. We ask that you set up your booth prior to the fair opening, either the
night before the fair. Friday, 12/5 the school will be open 4 to 7pm (please use the back/gymnasium entrance), or the
morning of the fair the school will be open at 7am and we ask that you are set up by 8:30am. The fair opens at 9am and the
fair closes at 3pm. AMS PTSO asks that you dispose of all trash in the proper receptacles and that you leave your area as you
found it. Booths are assigned on a First Come First PAID basis. Signed vendor applications and checks due by 11/21/14.
The Vendor agrees to hold harmless the Ansonia Middle School and AMS PTSO collectively or individually for any loss or
injury, that may arise or occur, by or to the independent contractor, his employees or agents, his property, or merchandise,
from any causes of negligence, natural or otherwise, during or while conducting any business or any other related activities
by himself or his agents, in connection with this agreement as authorized by the bylaws of the Ansonia Middle School PTSO.
