Why go from paper to electronic?

Enhance consistency of discharge process & efficiency (using LEAN principles)
Improve transitions of care, increase data portability (HIE)
Comply with federal mandate from CMS (ACA and JC)
Improve patient experience (legibility, comprehensiveness of documentation)
When/Where:
• April 29th house-wide rollout
• All med surg units affected (4B, 4D, 5A, 5C, 5D, 6A adult patients, 7D)
• Home med collection begins in ED
• ePDP/Discharge Instructions print on med/surg units
• ePrescriptions delivered to Discharge Pharmacy and community Pharmacies
• Providers complete all aspects of discharge process electronically, independent of
location
